Cultural questions for our non-US friends :)

Could any of you not living in the US answer these questions for my friend? We worked together at our old workplace and she's going back to school for nursing. In her one class, she must interview someone not from the US about their country's healthcare system. She doesn't know anyone outside of the US so I told her I might be able to help her out with my online buddies.
