It's screen 745 from PQ4. However, never having actually played the game, I can't identify what that screen is.

I'll make this easier to solve, I hope.

The area on the screenshot can be spotted after encountering the killer in PQ4.

If this still doesn't help, then I can make the hint a bit more clear, but it might need a spoiler tag for those that haven't played/completed the game.

Yeah, it's a large chunk of a completely nondescript image that could be anything from desert dunes to the inside of someone's intestinal tract.
